“Despite the efforts of healthcare staff, patients suffer harm every day in healthcare systems. It is our duty to learn from these patient safety incidents to improve patient care”
Hospice UK, 2024
“Because of our ageing population, the number of deaths in the UK is projected to increase by 25% by 2048. We need a national plan to ensure that everyone in the UK can die in dignity, and with access to the services they need.”
Toby Porter, CEO of Hospice UK 2024
“The Government must act urgently and radically to ensure that dying people are able to access safe and high-quality care on a 24/7 basis.”
Ruth Driscoll, Marie Curie, 2024
“When things go wrong, patients are at risk of harm and many others may be affected. For the staff involved, incidents can be distressing and members of the clinical teams to which they belong can become demoralised and disengaged. Overwhelmingly these incidents are caused by system design issues, not mistakes by individuals.”
Rachael Marchant, Medical Director at Havens Hospices, March 2024
